Assumption of Risk I understand that participation in athletic camps, training programs, clinics, strength and conditioning sessions, and related activities offered by Daytona Beach Christian University (DBCU) involves inherent risks. These risks include, but are not limited to, physical injury, illness, permanent disability, paralysis, or death, as well as property damage. I voluntarily and knowingly assume all risks, both known and unknown, associated with participation, whether arising from my own actions, the actions or inactions of others, the condition of facilities, equipment use, or any other cause. *

Release and Waiver of Liability: In consideration for being permitted to participate, I hereby release, waive, discharge, and hold harmless Daytona Beach Christian University, and their respective trustees, officers, directors, employees, coaches, volunteers, agents, representatives, and affiliates from any and all claims, demands, actions, or causes of action, whether known or unknown, arising out of or related to participation in any athletic camp or training activity, including claims based on negligence, to the fullest extent permitted by Florida law.*

Medical Responsibility & Authorization: I certify that I (or the participant) am physically able to participate and have no medical condition that would prevent safe participation. I authorize DBCU staff to obtain emergency medical treatment if deemed necessary. I understand that all medical expenses are my sole responsibility. Insurance Acknowledgment: I understand that DBCU does not provide health or accident insurance coverage for participants. I affirm that I have adequate personal medical insurance or accept full financial responsibility for any medical costs incurred.*

Conduct & Dismissal: I agree to comply with all camp rules, safety instructions, and behavioral expectations. I understand that failure to do so may result in immediate dismissal from the program without refund. *

Photo & Media Release: I grant permission for photographs, video, or audio recordings taken during participation to be used by DBCU and partners for promotional, educational, or marketing purposes without compensation.*

Governing Law & Arbitration: This agreement shall be governed by the laws of the State of Florida. Any dispute arising from this agreement or participation in DBCU and/or partner programs shall be resolved through binding arbitration in Volusia County, Florida, and I waive the right to bring a lawsuit in court, except to enforce an arbitration award.*
